Gum disease at dentist near Farnborough

A patient came to Time Dental for a comprehensive healthy mouth assessment.  He was aware that he had receeding gums and he also noted that they were bleeding when he brushed.  He had suffered from heart disease and was taking warfarin which helps to thin the blood.

On assessing his mouth we carried out a periodontal (gum) screening as part of his comprehensive healthy mouth assessment.  This involves using a special measuring instrument that indicates if there are any periodontal pockets.

Gum disease can be a silent disease, it is linked to heart disease, diabetes, strokes and premature births.  In the early stages of gum disease the gums become inflammed.  You may notice bleeding from the gums when brushing.  As the disease progress the plaque bacteria in the mouth gets between the gum and the tooth and starts to destroy the boney support around the tooth.  This leads to periodontal (gum) pockets.  Depending on the depth of the pockets the gum disease could be early, moderate or advanced.  The deeper the pockets the more advanced the gum disease.  Once the bacteria in the mouth gets to the point of destroying the bone this is an irreversible process.  The good news is that it can be stopped from progressing any further, and if the gum disease is caught in the very early stages it is a completely reversible. Gum disease can also cause gum recession, bad breath, abscesses and tooth loss.

To help treat this disease a course of periodontal therapy is prescribed.  This is carried out normally by the hygienist.  The Dental hygienist will take specific recordings of the amount of bone loss, assess the percentage of plaque bacteria and bleeding in the mouth.  With this information a tailored periodontal therapy course will be prescribed.  This involves fine tuning your home dental care routine, identifying risk factors, debriding under the gum line to disinfect and clear the bacteria in the mouth and provide continual support.

Healthy mouths by replacing mercury fillings at dentist near Farnborough

A patient came to Time Dental for a healthy mouth review.  She had not been to see a dentist for sometime and wanted to ensure her mouth was healthy.  She was aware that she had a number of old mercury fillings and did not like the look of them.

On assessing  her mouth her mercury fillings were in fact failing.  The larger mercury fillings had cracks down the tooth and decay had taken hold.  Mercury fillings weaken your teeth overtime, on placement more of the tooth needs to be removed to lock the fillings in which essentially weakens the tooth.  They also exapnd and contract and combined with the forces from chewing, minor cracks in the tooth occur.  Once this happens bacteria can seep into the cracks and start the decay process.  Often the filling can break or the tooth will break.

Catching this early is a great preventative way of keeping the tooth healthy.  Small to medium sized amalgam mercury fillings can be replaced with direct white composite fillings.  Once placed by a skilled cosmetic dentist not only will the tooth be restored to health, but it will look like a natural tooth again.  White composite fillings can last many years, they also bring the strength of the tooth back as they bond and stick to it.

2 steps to a natural smile at cosmetic dentist near Farnborough

A patient came to Time Dental having had 2 incisor teeth missing from her upper teeth.  She was only 16 years old and had never had her lateral incisors come through which made her feel very self conscious.  The spaces had closed but she was unhappy with the colour of her teeth and the fact that she looked “fangy” with her 2 incisors missing.

We discussed her options and the steps involved to help her feel more confident.  The simplest way to help her would be a 2 step process. The first step would be to whiten her teeth, and the second step would be to carry out composite bonding on her canines to make them look more like incisors.

Step 1 involved taking impressions of her teeth for her custom fit tooth whitening trays.  There are two main ways for teeth whitening; in-office or professional at-home tooth whitening.  The end result for both tooth whitening systems is the same, the main difference is the speed.  Professional at-home tooth whitening is much more gentle on the teeth, so she wanted to go for this option.  The teeth whitening gel is placed into the custom fit whitening trays and inserted in the mouth and kept in place for 40 minutes.  This process is then repeated once a day for 10-14 days.

Once the fresher appearance of the teeth have been achieved, step 2 involves composite bonding.  This is where composite is added to the canine teeth and bonded into place and shaped to look like incisor teeth.  Composite bonding is an excellent way of improving your smile.  With the current white composite materials a natural appearance can be achieved by a skilled cosmetic dentist.

A patient came to Time Dental looking for a cosmetic dentist and to provide dental care to ensure a healthy mouth.  They had looked us up on Google and had read our reviews.  She had not been to a dentist for a while and was looking to replace some of her old amalgam fillings.

On assessing her mouth she had a range of size of amalgam mercury  fillings.  The larger amalgams also had cracks running down the side of the teeth.  Mercury fillings actually weaken your teeth.  On placement of the mercury fillings the tooth is often over prepared to help lock the filling into place.  Over time mercury amalgam fillings expand and contract. Combined with the forces on chewing this can result in cracks occurring within the tooth.  The cracks then propagate and the result is either the tooth breaking, the filling breaking, or decay as a result of the cracks.

When amalgam fillings are at a certain size the cusps of the teeth are often the first to the at risk of breaking.  This is when onlays are recommended to protect the weaker cusps.  Crowns cover and protect the whole tooth by encapsulating it, onlays are lab made restorations that cover the biting surface of the tooth.  There are several advantages of onlays; the edges are high up on the tooth which allow you to keep the tooth plaque free; they can be made of ceramic which makes it very strong; they last longer than direct composite fillings; and they cover and protect the biting surface of the tooth.

Cosmetic dentistry with dental implants at dentist near Farnborough

A patient came to Time Dental to see how we could help him to smile again.  He had a number of crowns and bridges placed many years ago and a few of his crowns had fallen out.  His bridge also felt loose.  He wanted to know his options and how we could help him.  On further discussion we found that his previous diet was that of drinking lots of cola and eating sugary sweets. This lifetyle diet had now changed and he was looking to smile again with confidence.

On assessing his dental health.  All of his upper teeth had decay underneath the crowns and bridges.  Where the crowns had fallen out remained only stumps of decayed teeth.  Completely unrestorable.  We discussed his options:

1. The gold standard would be a dental implant fixed bridge.  This uses 6 dental implants and holds a fixed bridge in place that would help restore his bite and help him to smile with confidence.

2. An implant retained cosmetic denture.  This is a removable appliance that is supported by 4 implants that help to retain the cosmetic denture securely.

3. A cosmetic denture.  This is a removable appliance and aesthetically looks like normal teeth.

The first step would be to remove his decayed teeth and provide a temporary cosmetic denture while his gums healed.  Following that step the descision would need to be made as to whether the patient wanted something fixed or removable in the mouth.

Adult orthodontics for straight teeth at dentist near Farnborough

A patient came to see us at Time dental wanting to discuss her options for straightening her teeth with adult orthodontics. She was unhappy with her overlapping upper and lower front teeth and wanted to know how she could improve her smile.

Orthodontics is the movement of teeth.  There are many different appliances that are used in adult orthodontics.  When you mention orthodontics to people they usually think of teenagers and train tracks.  Things have moved on!

A patient came to see us looking for options to fill the gaps in the back of his mouth.  He had not been to see a dentist in 5 years as he had a bad experience at his last visit.  He found that chewing food was difficult as he had lost a number of his back teeth due to tooth decay and there was another large cavity in another back tooth.

On discussion with the pateint, he was looking for a fixed option to repalce the gaps in his teeth.  He was not keen on dentures and had heard about dental implants.  The first thing that we had to found out was why he was getting decay and what was causing his teeth to break.

We discovered that he had not been to see a hygienist in over 5 years and he had advanced gum disease in multiple areas of his mouth.  He also had a diet of hard food that resulted in his teeth breaking.  The first step was to stabilise his dental health.  Placing dental implants in a mouth with active dental disease is a receipy for disaster.

After intensive gum (periodontal) therapy to stabilise his dental health we would then look at his options for placing dental implants.

Dental implants are titanium roots that replace the roots of teeth that have been lost due to dental disease such as gum disease or tooth decay.  Implants have success rates of 95% and are excellent for replacing single or multiple teeth.  The dental implant is placed into the bone by a specialist implant surgeon.  A crown is then attached to the implant using special abutments.  The amount of bone present is crucial for the success of dental implants.  This is assessed by the implant surgeon.

Missing teeth and having gaps in your smile can have a real impact on your self esteem.  A new client came to see us at Time Dental with exactly that. 

He had porcelain crowns and bridges placed almost 10 years ago and a few months ago some of his back porcelain crowns started to come loose.  On further discussion with him he wanted a solution as his line of work meant that he was in public alot meeting new people and he wanted to smile with confidence.  He also told me that he previously had a diet of fizzy drinks and lots of sugary foods but only recently found out that it causes tooth decay around crowns.  This certainly explained why his porcelain crowns were falling out and why they had so much decay underneath them.

On examination we found that the decay was very extensive under his crowns and that they were unrestorable.  This meant that most of his remaining upper teeth were rotten and would have to be removed. We therefore  had to discuss different options to help him restore his mouth to give him a long term solution.

One option would be to provide immediate dentures.  Cosmetic dentures are very life like nowadays.  Individually tailored with fantastic aesthetics.  However these would have to be removed daily and cleaned.

Another option could be implant retained dentures.  This is where titanium roots are placed into the jaw which help to secure dentures in place.  Again this is a removable option but much more secure then normal cosmetic dentures.

A fixed option could be implant retained bridges.  This is where 4-6 implants are placed which hold a fixed bridge which looks and feels like normal teeth.  Dental implants have a 95% successful outcome and are an excellent choice for having fixed teeth again.

Invisible braces at dentist near Hampshire

A client came to see us yesterday unhappy about her smile.  She had jsut got married a few months ago, and when she looked back at her photos she noticed how crooked her top teeth were.  She did some research and wanted to know what the options were for invisible braces.

On assessing her mouth, she had quite significant overlapping, or crowding, of her top and bottom teeth.  Her her upper teeth were also twisted and rotated out of position which really bothered her.

We discussed her options.  One option was to have traditional train-track braces but using tooth coloured brackets.  This was an option she was still not willing to consider, especially when her main concern were just her top front teeth. 

An other option would be to use Invisalign the clear aligner system.  These are removable clear aligners which are virtually invisible.  Invisalign is a great system but like with all treatment options there are limitations.  Due to the severity of crowding her treatment could take 1-2 years of active treatment. 

A further option was using the Inman aligner.  This is an appliance which again is removable and is made of clear acrylic and can move teeth in a matter of weeks.  It works really fast due to the gentle constant pressure it applies to the teeth from coiled springs.  Discomfort is very minimal and normally lasts only a few days after first wearing the appliance.  It is designed to move only the front four incisors and so was another great option for her.

One of the top reason why 50% of the population do not see a dentist regularly is due to the fear of pain.  However one patient did not like to see the dentist because he had a very sensitive gag reflex and could only have treatment under sedation.

A gag reflex is a very important safety mechanism.  It can help to stop food/ foreign objects from choking you.  However some people have an overly sensitive reflex, so having dental treatment can be a very daunting prospect.

At Time Dental, Dr Rashid first discussed the concerns that this particular patient had.  He had also had a very bad experience in the past as a child which had really put him off seeing a dentist.  He felt he had a loss of control whenever he was in the dental chair.  The first steps to help him was to give him back the sense of control, and also guide him through gentle breathing excercises and techniques to help him control both his feelings of loss of control and his gag reflex.  This was all before he even sat in the dental chair.

A comprehenisve dental examination and consultation was carried out.  This is where his teeth, gums, tongue, tissues that line the mouth, chewing muscles and jaw joint were assessed for health.  X rays were also taken to assess the invisble health of the teeth.

Due to the particular apprehension of the patient a joint decision was made to carry out his dental treatment under conscious sedation.  This uses a sedative which helps to relax the patient.  Although the patient is completely awake through the treatment the sedative also provides excellent amnesic properties.
